Chapter 1 - Project Overview
Chapter one discusses the prevalence, cost, and complications associated with Chlamydia trachomatis in the United States. This chapter also discusses the formation of the Infertility Prevention Project (IPP) as a response to reduce morbidity and subsequent complications associated with Chlamydia trachomatis, and the formation of the IPP in Region VIII.

Chapter 2 - Regional and National Overview
This chapter gives an in-depth view of the IPP project in Region VII, including regional goals, committee structure, overall objectives and contact information. There is also a section detailing the background and goals of the National IPP, as well as partnerships, infrastructure funds, and innovations.

Chapter 3 - Project Requirements
This chapter discusses the requirements of IPP participants. It explains the makeup of the Regional Advisory Committee (RAC), as well as the participants' role in the (RAC).

Chapter 4 - RAC Structure
Chapter four outlines the structure of the IPP RAC committee. In addition to having a helpful flow chart outlining the entirety of the IPP, it also details the structure of the Executive Committee and the RAC Executive Committee Duties.

Chapter 5 - Region VIII RAC Roster
Chapter five contains a list of the IPP Region VIII RAC members, Ex Officio members, and Affiliated Public Health Representatives. For each person, this chapter contains the person's name, title, address, and phone and fax number.
Chapter 6 - Infrastructure Objectives
Chapter six is the infrastructure application that JSI submits to the CDC detailing the activities of the Region VIII IPP project during each budget period.It includes the regional plan for Region VIII as well as data collection from the region.

This chapter contains the workplans for Region VIII's three work groups: Laboratory Services, Data Use/Effective Use of Resources, and Clinical Services.

Chapter 8 - Data Form Instructions
Chapter eight contains detailed directions on how to complete data forms used to report IPP data. Chlamydia project data are used to evaluate the program and are reported to JSI, who then compiles the data and reports it to the CDC.
